Medical Requirements Physical Examination Record from any Indian Government/State level Hospital. The photo on the Record should also be stamped by the Hospital. Valid for six months from issued date. Original and Photocopy.

The invitation should also mention the name and passport number of the applicant, with signature of Authorised signatory and Company Seal.
  • Return Confirmed Ticket with Airline Stamp.
  • Passport copy of front page and last page.
  • Personal Original Bank Statement (for last 6 months) with minimum balance of Rs. 1,60,000.00 with Bank Seal and authorised signatures.
